Bridging Care Between Visits

Many patients leave appointments understanding what matters for their health, yet find it difficult to carry those recommendations into the realities of everyday life.

Stress, competing responsibilities, longstanding habits, uncertainty, and the demands of daily life can all influence whether change takes hold.

Clear Thread Coaching offers evidence-based, HIPAA-informed health coaching that helps patients slow down, overcome barriers, and turn clinical recommendations into realistic, sustainable action.

How coaching supports care

Coaching complements medical care by providing structured support between visits.

I combine reflective conversation, Motivational Interviewing, and evidence-based behavior change strategies to help patients move from understanding what matters to putting it into practice.

Patients are supported to:

  • translate clinical recommendations into realistic daily actions

  • identify manageable next steps

  • explore barriers to follow-through

  • recognize patterns in habits, stress, and motivation

  • build confidence through small, sustainable changes

Sessions are collaborative, patient-centered, and tailored to each person's real-life context.

Collaboration & Referrals

With patient consent, I collaborate with referring providers to support continuity of care.

Patients participate independently through secure virtual coaching sessions while continuing their medical care.

  • "I was overworked, overweight, and over-committed. It was time to get my priorities straight. My wellness journey began by having the courage to admit I could not make the necessary changes alone, I needed a trusted professional to walk this journey with me. That’s when I was fortunate enough to have met Rebecca and change the direction of my life and begin choosing a healthier well-being and lifestyle. We met regularly and focused on establishing realistic and obtainable goals to foster a healthier lifestyle… I could finally show up personally and professionally stronger than ever."

—Chad D., V.P. Strategic Relationship Management

  • "Rebecca was instrumental in helping me find ways to cope with some work-related anxiety. By teaching me ways to use mindfulness meditation, I was able to be present in the moment and feel calm, and not let my emotions get in the way. Her true power is in the skill of listening, then understanding, then finding ways to help."

—Jim L, Retired Producer, Videographer, & Editor

  • "Rebecca has gotten to know my strengths, weaknesses, and tendencies, which has made her very effective at asking me thought-provoking questions that push me to see different approaches than I would think of by myself. She has helped me stay accountable to myself and to some concrete goals by encouraging me to make pragmatic plans and adjust as needed. I greatly appreciate the curious and empathetic approach she takes in helping me think through my current challenges. "

    —Jessica S., Research Analyst

  • "My engagement with Rebecca exceeded expectations. Great listener, challenged at the right level, makes the main thing the main thing. She coached me as I developed my plan, but she helped me look at the opportunity from different perspectives and kept me accountable. You have to put in the work but she can be a catalyst to help you focus on the things that matter."

    —David B., Retired Executive Advisor

  • "Rebecca has a great way of encouraging people to open up about goals, concerns, wins and losses (something that does not come naturally to me). Her approach also naturally lends itself to ongoing introspection, and I found myself looking forward to our sessions to help me hold myself accountable to the actions and outcomes that I had called out in our earlier sessions."

    —Louis B., Senior Commercial Manager

  • "Rebecca’s coaching made a profound impact on my career journey. She helped me uncover my strengths, develop the confidence to lead, and stay disciplined in pursuing my goals. Thanks to her guidance, I had the courage to apply for a management role — and I got it! Her support was invaluable."
